Organ Donation Pledge Terms & Conditions

By pledging to be an organ donor, you acknowledge and agree to the following:

• I voluntarily pledge to donate my organs/tissues after my death for the purpose of transplantation.
• I understand that my pledge is a statement of intent and not a legally binding contract.
• I confirm that I am 18 years or older and of sound mind while making this pledge.
• I understand that the final decision regarding organ donation will be made by medical professionals at the time of my death, based on medical suitability.
• I agree that my decision to donate will be respected, but it may be overridden by my next of kin at the time of my death.
• I commit to informing my family members about my decision to donate organs.
• I understand that organ donation will only occur after I am declared legally dead by appropriate medical authorities.
• I agree that my personal information will be kept confidential and used only for organ donation purposes.
• I can change or withdraw my pledge at any time by contacting Shanti Dehdan Charitable Trust.

Medical Confidentiality: All medical information provided will be kept strictly confidential and will only be accessible to authorized medical personnel involved in the donation process.
